The Sub-Acute Nurse Manager provides direction and coordinates care for 60+ (approximately half) of the Oak Brook Care’s patients. This is a dynamic role where patient admission and discharges are high. The influence of this specialized Nurse Manager is expected to grow as the older US population increases.
SUB-ACUTE NURSE MANAGER RESPONSIBILITIES
The Sub-Acute Nurse Manager is responsible for the clinical, financial, and quality management on the sub-acute unit. He or she will create a caring environment that focuses on the individual needs of patients. In addition, he or she communicates with families and creates an open and positive environment. He or she also evaluates all new patients and ensures that the plan of care is implemented.
Inside the facility, the Sub-Acute Nurse Manager plays an important role in the communication with other departments in the facility. He or she provides direct supervision of all nursing staff on the unit. He or she works with the Director of Nursing to identify the educational needs of the staff and develops an ongoing educational plan. The Sub-Acute Nurse Manager will partner with the Social Work and Therapy departments to establish appropriate patient discharge plans. The Nurse Manager attends weekly Medicare, Rehab, and Care Plan meetings to establish patient goals and discharge plans. He or she also attends QAPI, JCAHO, UR and compliance meetings and contributes related audits and reports
Outside the facility, the Sub-Acute Nurse Manager represents Oak Brook Care with the hospitals and discharge planners. We use and teach INTERACT® for quality improvement and readmissions rate reduction initiatives. Fosters an environment of collaboration and respect with specialty physicians, including our on staff cardiologist, pulmonologist and physiatrist.
SUB ACUTE NURSE MANAGER QUALIFICATIONS
- Graduate of accredited School of Nursing – BSN preferred
- Current registration with Illinois Department of Professional Regulations
- At least one year experience in Nursing Service Administration, Geriatric Nursing, or long–term area setting
- Supervisory and leadership experience
- Full time position with some weekends
